Services we offer:

Meals
Meals are provided to all seniors 60 and over, and their spouses, at the 15 mealsites located in Jackson and Josephine Counties. Home delivered meals are also provided to homebound seniors 60 and over as well as adults with disabilities who meet eligibility criteria.

Information and Assistance
Helping seniors and person with disabilities find the assistance they need.

Case Management 
Working with clients and their families to integrate social, financial, and health care options into individualized care plans and authorizing payment for long-term care for people eligible to receive financial assistance.

Protection from Abuse 
Investigating reports of abuse; follow-up services and consultation to prevent further abuse.

In-Home Care
Paying for help with personal health care, hygiene, and (in some cases) with basic housekeeping so eligible vulnerable people can continue to live independently in their own homes.

Community Based & Institutional Care 
Paying of foster home, residential care facility and nursing home care for Medicaid-eligible elderly and adults with disabilities.

Pre-Admission Screening
Helping individuals and their families decide the most suitable option when nursing home care is being considered.

Food Stamps
Determining eligibility for food stamps, an electronic benefit transfer card for supplementing food purchases.

Medical & Cash Assistance
Determining eligibility for help with medical costs and, in some cases, living expenses for seniors and disabled adults. Medicaid and Oregon Health Plan assistance for eligible persons ranges from Medicare premium payment to help with paying for doctor and hospital visits, dental and prescription costs.

Medical Supplies
Paying for medical supplies and equipment for Medicaid-eligible persons.

Foster Home Licensing
Inspecting and licensing adult foster homes in Jackson and Josephine counties.

Risk Intervention
Case management for adults who do not meet income or resource tests, and who are at risk of going to a nursing home within one year because of health or other problems.

Gatekeeper Program
Training employees of community businesses to recognize changes in customers appearance and behavior that might indicate a need for help.
